Our digital health programs give you easy access to mobile applications to help you achieve the your health goals from the comfort of your home.

How to enrol in our programs

Our digital health programs are funded by participating private health insurers, if you meet the appropriate eligibility criteria. Eligibility criteria and participating funds differ between programs.

‎Cardihab

Honeysuckle Health has partnered with Cardihab to deliver digital cardiac care programs, empowering individuals to manage their heart health from the comfort of their homes — at no additional cost to eligible members or participating funds.

These programs combine the convenience of digital delivery with the support of experience Honeysuckle Health clinicians, aiding long-term heart health management.

Cardihab:
Cardiac Rehabilitation Program

This 6-week evidence-based program is designed for individuals recovering from a recent heart-related event. It adheres to Australian Heart Foundation and Australian Cardiac Rehabilitation Association guidelines, complementing GP and specialist care.

Weekly one-on-one phone calls with a qualified clinician.
SmartCR mobile app for tracking medications, blood pressure, symptoms, step count, and more!
Access to educational resources and wellbeing support.
Tailored exercise program to support recovery and health goals.
Available to nib, Bupa, and St Lukes members (eligibility requirements apply).

Cardihab:
Primary Prevention Program

This program focuses on improving heart health by reducing the risk of developing heart disease, or preventing heart disease from worsening.

It promotes lifestyle changes through education, monitoring, and ongoing personal support from experienced Honeysuckle Health clinicians. It also helps you keep on track with prescribed medication through reminders and tracking.

Regular personal check-ins with qualified Honeysuckle Health clinicians
SmartCR mobile app for tracking of exercise, health metrics (Blood pressure, stress, heart rate etc), and symptoms.
Education on improving risk factors, diet, physical activity, and mindfulness.

Available to Latrobe Health Services members (eligibility requirements apply).

Cardihab for Providers

Program type: Digital Cardiac Rehabilitation program designed to improve patient outcomes and cardiovascular health.

Suitable conditions: An eligible member managing heart disease with a recent acute presentation or heart procedure. The program is available to eligible health fund members aged 18 years or older.

Silvercloud® by Amwell®

Designed to help you manage your mental health and wellbeing, the SilverCloud® platform gives you flexible access so you can interact where you feel most comfortable. The SilverCloud platform uses evidence-based methods such as Cognitive Behavioural Therapy (CBT), mindfulness and positive psychology to support and encourage positive behaviour change and mental wellness. SilverCloud programs are intended to complement the care you receive from your GP, psychologist or psychiatrist and includes:

SilverCloud programs consist of six to seven modules. Each module features interactive and engaging activities, applications, and quizzes. You can choose to interact with the platform on your own, or with the support and encouragement of a Honeysuckle Health Mental Health supporter.
