Supporting people who have had, have, or are at high risk of having diabetes, cardiovascular disease, heart failure, and or chronic obstructive pulmonary disease. This service is a phone-based program that gives you access to your own personal ‘coach’ who are giving you access to qualified dietitians, pharmacists, or registered nurses. Designed to accompany the usual care provided by your health care team, the program will include:
The COACH Program will empower and support you to prevent chronic disease development or progression. Although program length varies according to individual needs, most people can expect there to be 5 to 6 calls over a 6-month period.
